COVID-19 EOC Logistics Request Form

  1. EOC Requests Form 213 - COVID-19 Response
    This form is to submit a request through the Town of Windsor Emergency Operation Center in response to the COVID-19 declared emergency. It is for employees and Town business only. While Logistics will attempt to fill all requests, they are not guaranteed. All requests must be approved by the Logistics Section Chief prior to fulfillment.
